Zaw Added 14,000 ICD9 codes
- truncate to one decimal place, don't round
- chart view skinning
- warning window when deleting
- once locked, only deletion by author
- if fields in SOAP note are empty, delete it.
- Give reason for deleting
Mike is refactoring data and creating model so no data will ever be lost

Add field for deceased, moved etc.
